An exciting opportunity has arisen for a part-time permanent Band 6 Occupational Therapist to join our innovative Healthy and Active Rehabilitation Programme (HARP) team. You would ideally possess proven skills in working with people with multiple long term conditions as well as have experience in assessment evaluating patient progress and planning treatment programmes in group situations. The post will involve negotiating behaviour change using goal setting during all stages of the patient journey.
You will work in a team that embraces supported self management lifestyle modifications and individualised goal setting. You will provide direct clinical care and treatment and will plan and provide patient education sessions mainly in an out-patient and community setting. Knowledge and experience of working with a complex multi morbidity population an advantage.
You will contribute to the working of the multi-disciplinary team and the training and development of rotational staff students and assistants.
Clinical and line management supervision will be provided and we are keen to support applicants to learn and develop skills within the role.
A Degree/Diploma in Occupational Therapy and HCPC registration are essential requirements.
A current driving licence and access to a car are essential for community working.
