An exciting opportunity has arisen for highly motivated, skilled and enthusiastic Registered Mental Health Nurses to join our busy Community Mental Health Team based at Chester Le Street Health Centre. You will be expected to provide an assessment/treatment and care for adults with variety of mental health conditions (affective and psychosis in nature).
You will need to have significant experience of working with adults with mental ill health in their own homes and care setting, delivering clinical interventions specific to care pathways. You will need to possess very good communication and interpersonal skills. You are required to have the ability and experience to manage complex cases in partnership with other agencies providing care coordination in line with The Care Program Approach framework. You will support service users in their recovery. As a Senior Practitioner you will be expected to supervise and mentor junior members of staff as well as student nurses. You are required to use your own initiative and make decisions independently for this post.
There is an expectation of commitment to the development of high quality care, and to engage in quality improvement activities. You will be supported with regular management and clinical supervision. We use Cells, Daily Decision meetings and Complex case discussions to ensure our staff are fully supported by colleagues and Leadership Team. You will be offered training and development opportunities to fully support your role within the team.
Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
