Senior Case Manager

Job Purpose

Proactively manage and support the rehabilitation of service users to contribute to the overall objective to reduce reoffending across the Community. This role will cover Scarborough and York.

Rehabilitation Company (CRC). Ensure the contract objectives and sentence of the court are delivered through the Interchange Operating Model, including group induction, assessment, management of risk, supervision planning, commissioning and delivery of interventions. Demonstrate a personalised approach with a focus on enablers and strengths of the individual. Support the Flex Team to deliver objectives by managing complex/higher risk cases and overseeing/supporting Case Managers.

  • Proactively engage with service users with the aim of sustained rehabilitation
  • Manage a caseload of the more complex/higher risk cases based on evidence-informed assessment(s), including Risk of Serious Recidivism (RSR) score
  • Demonstrate and provide evidence of understanding protected characteristics and working to meet specific requirements
  • Monitor service user progress for the duration made by the sentence of the court
  • Comply with and contribute to children and adult safeguarding duties, adhering to national and local policy and practice guidance promoting the welfare of children and adults, and alerting relevant colleagues and stakeholders should any risk be identified
  • Provide practice oversight and guidance to Case Managers as required (informal coaching and mentoring)
  • Demonstrate collaborative working within the Flex Team; sharing practice and skills, and promoting a team ethos
  • Proactively take responsibility for developing and maintaining relationships and networks, including but not limited to Multi-Agency Risk Assessment Conferences (MARAC)and Domestic Violence (DV) multi-agency relationships, to ensure appropriate support and engagement with service users
  • Actively manage risk and take responsibility for recognising when risk escalation is required and take the necessary action
  • Contribute and supply information for the court as required, e.g., breach or not guilty plea
  • Responsible for recording and record keeping in line with company policy and procedures
  • Perform other duties commensurate with the post as from time to time may be determined by the line manager, Director or above

Operational Planning and Management

  • Ensure that all case-management activities will be undertaken in an enabling manner in line with rehabilitation goals to enhance consistency in delivery of the sentence/contract compliance
  • Inform CRC commissioning strategy implementation by providing feedback on service delivery and service user requirements as appropriate
  • Responsible for motivating and monitoring the service users’ progress through the Interchange Modules and the elements of their support package
  • Demonstrate Skills for Effective Engagement, Development and Supervision (SEEDS) in day-to-day practice
  • Delivery of the Interchange Model (including the six core modules) as follows:

Interact (Assess-sentence plan-implement-review and evaluate- enforcement and referral)

  • Deliver/co-deliver individual and/or group induction; explain (i) the Interchange Model (ii) expectations of sentence and possible consequences of non-compliance; support service user self-assessment
  • Conduct strengths-based assessment linked to co-development and review of the sentence plan

Plan and commission services to enable change

  • Maintain clear and timely record-keeping on the Case Management System
  • Access as appropriate the Directory of Services aligned with personalised budgets
  • Monitor progress through Record of Achievement
  • Where necessary commence prompt enforcement action

Intervene (Interventions used to support change and deliver the sentence)

  • Refer and signpost service users to relevant supplier in the Directory of Services aligned to a personalised budget
  • Identify and utilise relevant services for protected characteristic groups
  • Identify interventions that promote change and personal development

Integrate (Activities designed to engage offenders with their community and access its resources)

  • Understand, engage with and influence local community partnerships for the benefit of service users and local communities
  • Optimise Networks towards sustained rehabilitation
  • Participate in multi-agency forums

Accredited and Non-Accredited Programmes

  • Motivate service users to engage, comply with and complete accredited programmes and other group work

Finance and Commercial

  • Manage the service users’ budget when developing their personal rehabilitation plan
  • Support 3SC in the evaluation of supply chain effectiveness
  • Ensure performance metrics are achieved through the recording of information in a timely manner, in line with practice guidance and through working as a member of the Flex Team to proactively understand and ensure that the performance and quality standards are met; to include providing oversight and guidance to Case Managers and Case Coordinators regarding performance and quality, under the direction of the Interchange Manager

Decision Making

  • Use professional judgement whilst exercising all case management duties and assessing and managing escalating risk appropriately with auditable evidence in place
  • Provide practice oversight and guidance to Case Managers (not line management)
  • Support Case Managers to use professional judgement and effective decision making

Relationships

  • Foster a positive working relationship with local partnerships, suppliers of interventions and other community resources
  • Provide agencies with relevant service user information, where appropriate, including sentence details, risk, and arrangements for addressing non-compliance
  • Collaborate with the National Probation Service (NPS) where appropriate
  • Collaborate with others to solve problems and improve ways of working
  • Undertake peer review as part of quality assurance (QA) role
  • Build an effective working relationship with the service user adopting a strengths-based approach
  • Build an effective working relationship as a member of the Flex Team and other colleagues
