Cartref Homes are a small and highly regarded service provider supporting individuals with learning disabilities and additional and complex needs including forensic histories.
This is an exciting opportunity for qualified nurse (RNLD/RMN) to develop and manage an innovative specialist supported living service supporting individuals being discharged from hospital under the Transforming Care agenda. It will be a key appointment for the company as it seeks to develop and grow its services.
You will be responsible for delivering a high-quality service provision in a specialist living service for adults with learning disabilities and/or mental health difficulties (and associated conditions), who have a forensic history.
As manager, you will be directing and coordinating a multi-disciplinary approach to meeting the needs of individuals with complex presentations, ensuring their aspirations and goals are met through person-centred planning and a commitment to evidence-based therapeutic interventions that apply the principles of positive behaviour support.
You will lead a team of support staff working alongside various health and social care professionals and clinicians, developing treatment and support strategies designed to prepare and equip individuals to live safe meaningful and valued lives in the community. You will need to develop effective and positive relations with commissioners, care management and clinicians.
As manager of this innovative supported living service you will need to achieve registration with the Care Quality Commission (CQC) and ensure that the service adheres to the regulations and standards that apply.
The successful candidate will have a BSc. or Dip HE in learning disability or mental health nursing with a current registration with the NMC along with a minimum of 5 years experience working in learning disability services. They will have to demonstrate knowledge and understanding of the current theory and practice relating to the service provision for people with learning disabilities.
There is also a need to possess a current full driving licence.
Working hours will be 37.5 per week.
