A Qualified Social Worker is required to join a busy Hospital Social Work team based in Huddersfield, West Yorkshire.
Working within the Hospital Social Work team you will have the following responsibilities;
*Effectively manage a complex caseload with appropriate supervision, guidance and support. The role holder will be accountable for highly complex decision making across a range of social work duties and responsibilities.
*Facilitate safe and timely discharge of patients back into the local community.
*Prepare and produce a variety of high quality and professional written documentation on behalf of the Council in accordance with national and local policies and procedures and within agreed timescales.
*Work in conjunction with and providing consultation to partner agencies to deliver effective planning for Adults, their carer's and families to ensure positive outcomes.
*Ensure that the practice of ethical record keeping is observed and recording is evidence based, accurate and up to date.
To apply for this role candidates must -
Hold a degree, post graduate or Professional Qualification in Social Work
Hold full HCPC registration
Ideally have 2 years post graduate experience in the relevant team
Have Clear and valid DBS disclosure
Have access to their own car
