Your main roles and responsibilities will include:
To work in partnership with young people as they leave care and move onto adult life.
The Leaving Care Worker role requires you:
To give practical support, advice information and guidance to young people 17 to 25.
To coordinate services via Pathway Planning to help young people access the services they need to move from care to adult life
To help young people have access to Suitable Housing and Employment Education and Training opportunities.
To work in partnership with colleagues from Children’s Services and other appropriate agencies
Initial Specific Responsibilities
You will be the allocated personal adviser for a group of care leavers, some of whom face complex difficulties.
You will be expected to travel in and out of county so you need a full driving licence and daily use of a car.
Some evening work is inevitable. Occasional weekend work may be required too.
You will be line managed by experienced Team Managers who are HCPC registered social workers.
