Primary Care Network Social Worker

The Primary Care Networks Northern team are a supportive, diverse and high performing team of 18 staff, based in the North of the County (East Grinstead / Burgess Hill / Haywards Heath / Horsham / Crawley). We strive to work collaboratively with MDT colleagues preventatively and in a strengths-based way with Adults with long term health conditions at high risk of admission to hospital and/or who have high use of NHS services.

This includes complex casework, Safeguarding, and work under the Mental Capacity Act, with people with physical disabilities which have onset in adulthood and /or issues such as self-neglect, hoarding and alcohol misuse, as well as older people with dementia. The MDT's consist of Team Lead, Clinical Lead, Community Nurses, Psychology, OT, Physiotherapy, Mental Health Practitioners, Social Workers, Care Co-ordinators and Age UK.

Responsive Neurological Services Social Worker

Our highly regarded Responsive Neurological Services MDT, based at Horsham Hospital is currently looking for an experienced Social Worker / Senior Social Worker, covering the North of West Sussex thus requiring significant travel around the area. This includes Horsham, Crawley, East Grinstead, Hayward Heath, Burgess Hill and the surrounding areas.

This unique role has become available and we are looking for a candidate who is passionate about strengths-based working, rehabilitation of acute conditions such as stroke and ABI, and management of progressive conditions such as Parkinson's, MS and MND, with relevant experience of MDT working and/or working with Adults with neurological conditions. You will be working with a specialist MDT including Team Lead, Clinical Lead, SALT, Psychology, OT, Physio and support workers.


Primary Care Network Social Worker

We are seeking an enthusiastic Social Worker to join the team, and to be based in one of the PCN MDT's. You will have good communication skills and be able to develop relationships with your MDT colleagues, attend MDT weekly meetings, GP meetings, and keep up to date records across WSCC and NHS IT systems in addition to managing a caseload and covering Duty. The Team have a strong sense of teamwork and will support you with your developmental needs and career progression whilst we continue to deliver a good quality service to individuals in conjunction with our NHS and third sector partners.

Responsive Neurological Services Social Worker

We are seeking an enthusiastic Social Worker to join the team, ad to be successful you will need to evidence the ability to work autonomously under pressure, dealing with Early Supported Discharges from Hospital, but also to work collaboratively with your MDT regarding long term planning and rehabilitation, participating in Clinical Lead meetings with NHS colleagues, and keeping up to date records across WSCC and NHS IT systems.
