Job overview

This is an exciting opportunity for a full-time or part-time Practice Nurse to work across our three surgeries in Leicester. The practices are currently looking for an experienced, motivated and enthusiastic Practice Nurse to join our friendly and professional team. We are looking for someone who will share our commitment to providing excellent health care to our patients.

Main duties of the job

You will already be a Practice Nurse who has a range of clinical skills, as well as the experience and ability to competently deal with patients who present to the surgery. The practice nurse will have extensive experience working in General Practice and chronic disease management.

You will confident to work within the policies, refer on to other disciplines when necessary, or consult with other colleagues, or GP, when appropriate. The post holder will support patients to be healthy, monitor those with long term conditions and deliver health prevention, promotion and screening activities.

This will include but is not limited to assessing patients, planning and implementing treatment protocols, reviewing and tracking progress.

The practice nurse will work collaboratively with the practice team, and wider clinical and non-clinical teams across DHU performing tasks as required by the management team, the organisational policies and procedures.

A commitment to a quality service, excellence in clinical practice and continuous professional development are integral to the role and the development of the service.

The post holder will participate in promoting and advancing DHU Health Care CIC.
