The Intermediate Care Team is a multi-agency team working together to offer an alternative to hospital admission by maintaining people in their own homes and to facilitate the smoothest and earliest possible discharge from hospital. The multi-disciplinary team provides assessment short term intensive rehabilitation and homecare packages. We have opportunities for highly motivated individuals to join our busy enthusiastic team committed to delivering quality service working closely with colleagues across professions and agencies at this exciting time as we remodel the service.

A diploma/degree in physiotherapy is essential as well as extensive clinical experience and evidence of continuing professional development. You will have the opportunity to develop competencies in Nursing Occupational Therapy and Social Care Management working in an inter-professional manner.
The ability to develop communication skills as part of a wider communication team knowledge of Join Future Agenda Older People Services Care Management and the rehabilitation Framework is essential along with a genuine interest and understanding of inter-professional and inter-agency working.

Hours: 37.5 per week

This is a fixed term/secondment opportunity for one year from date of appointment.

A car owner/driver is essential.
