Part of a multi-disciplinary team, with responsibility for areas of chronic disease management and undertake clinical medication reviews to manage patients with complex polypharmacy
Provide primary support to general practice staff with prescription and medication queries, and the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice
Provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services
Lead on delivery of PCN DES national service specification for structured medication review and optimisation and support Enhanced Health in Care Homes, CVD prevention & Diagnosis
Support a team of clinical pharmacists across general practice, supported by a senior clinical pharmacist
