An exciting opportunity to work on the Coast in the East Riding of Yorkshire as a PCN Clinical Pharmacist with the Bridlington Primary Care Network (PCN).

Bridlington Primary Care Network (PCN) is looking to recruit a PCN Clinical Pharmacist to support the patient care in the Primary Care Network in line with the new Network Contract Directed Enhanced Service (DES) Specification.

The PCN comprises of 6 committed practices with a population of around 41,000 and this role will form an integral part of the PCN Leadership Team.

You will be supported to develop your role to become a non medical prescriber if that qualification is not already held and will be required to enrol on the General Practice Pharmacist Training Pathway CPPE, unless an equivalent qualification is already held or exemption is agreed by CPPE. You will be a great team player, but capable of independent, autonomous work. You will have excellent inter-personal skills and ideally experience of developing new services to adapt to the evolving needs of the PCN Directed Enhanced Service (DES). You will have excellent communication skills and be committed to providing exceptional patient care. This is a new role and will develop over time, but the expectation is you will work autonomously in the multi-disciplinary team, supporting PCN aligned care homes, supporting long term conditions effectively through complex polypharmacy, medicine optimisations and strutured medication reviews.

The post holder will ensure that the PCN integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.

As a senior member of the PCN Team you will be expected to provide leadership and mentoring to any Pharmacist or Pharmacy Technician employed by the PCN.
