Home ward Ealing is an integrated health and social care service. The service helps people to receive treatment safely at home during a period of acute illness or to receive a short period of additional intensive support after hospital admission.
West London Mental Health NHS Trust is the lead provider for this service, in partnership with Central and North West London NHS Foundation Trust, Chelsea and Westminster Hospital NHS Foundation Trust, London Central and West Unscheduled Care Collaborative and the London Borough of Ealing. Home ward is a physical health service and sits within the Liaison and Long Term Conditions service line, within WLMHT's Local Services.
The role of the service is to deal with acute illness and rehabilitation, preventing admission to hospital and also facilitating complex discharges from hospitals, providing care within the patient’s home. A high level of skill is required to manage the risk of supporting patients and delivering their rehabilitation needs in their own homes and also supporting the multi-professional team with understanding Occupational Therapy needs for this group of patients.
An exciting opportunity has arisen for an experienced full-time Occupational Therapist within the Short Term Rehabilitation and Supported Discharge Team.
There will be opportunities to participate with practice placements of OT students, help develop the service for the benefit of patients and work closely with all members of the multi-disciplinary team including doctors, nurses, physiotherapists, social workers, care coordinators and pharmacists.
The successful candidate will also be expected to provide supervision and annual appraisals to other members of the team in order to ensure quality assessments and interventions across the service. You will also receive regular supervision, support and annual appraisal from your line manager as well as peer support across the service. Regular training and updates are provided as part of this role, as well as opportunities to develop your skills and expand your role within the team.
The role is full time and includes weekend working on a rota basis (currently 1:12). There is also a need to use your car to visit patients in their own homes.
