Night Senior Care and Support Assistant
Salary: £11.00 per hour
37.5 Hours per week.
Based: Colchester and surrounding areas

Swan Care Services are looking to appoint a full time passionate and motivated Senior Care and Support Assistant to work as part of a supportive team to deliver Rapid Response services to people living in the community.

Our team enables people to be discharged safely from hospital back to their own home and provides care and support service to people to avoid them being admitted into hospital, no two days are the same!

You will be responsible for supervising a small care team ensuring quality and compliance. You will be meeting customers as they are discharged from hospital, carrying out assessments, developing care plans, scheduling visits for clients and providing practical support.
