Hywel Dda Health Board are currently looking to recruit a full time Mental Health Practitioner to join the Integrated Community Mental Health Team, based at Hafan Hedd Resource Centre, Newcastle Emlyn covering the south of Ceredigion area.

Our services work through an Assessment & Recovery approach with people accessing secondary mental health services.
The post holder will be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care for a defined clinical caseload including the role of a Care Co-ordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.
The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies. maintaining good communication across all boundaries.
Hours are to be full time / part time / various / job share
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply
