Health and Wellbeing Coach - Macmillan Community Nursing Service – West Cumbria
Band 3, total 46.75 hours available, fixed term / secondment
An exciting opportunity has arisen for a number of Band 3, Health and wellbeing Coaches to support the Lead Cancer Nurse to deliver a Macmillan Community Cancer Nurse service to provide bespoke care for cancer patients across West Cumbria. The posts will be offered as a 12 month secondment opportunity or on a fixed term contract.
Macmillan and North Cumbria CCG have invested funding into the North Cumbria health economy to create a Community Cancer Nursing Service for the people of West Cumbria, to provide an integrated care approach and support individuals living with a cancer diagnosis to have access to cancer care and support closer to home.
Based in primary care and employed by NCIC, working across the West Cumbria Primary Care Network, you will work in partnership with primary and secondary care health care professionals, acting as the link person for our patients and carers. Your role will involve working within a multi-disciplinary team based in each Primary Care Network, your duties will include:
- Provide support for all cancer patients following a diagnosis
- Signposting to services
- Promoting health and wellbeing
- Supporting Health and Wellbeing events
- Work closely with acute oncology and cancer care coordinators in hospitals
- Working with DNs GPs and community teams/ Liaise with LD and MH services
- Coordinating care needs
- Fast track referrals to Secondary care as required
- Track patient pathway
- eHNA (Electronic Holistic Needs Assessment)
- Referral to appropriate support services
- Skills in managing SACT side effects (skin/mouth care advice)
- Competent in Central Venous Line Care (Dressing)
We are therefore looking for highly motivated individuals with excellent communication skills, who can demonstrate effective time management and organisation skills. Previous experiences of working with cancer patients are desirable.
The posts are available initially on a 12 month secondment / fixed term opportunity, however we do hope to secure further funding to be able to continue the service further. The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows if applicable to the post:
Standard Check: £26 + Administration cost of £4.20 = deducted at £10.20 from 1st month salary and £10 deducted from salary over following 2 months.
Enhanced Check: £44 + administration cost of £4.20 = £15.20 deducted from 1st month salary and £11 deducted from salary over following 3 months.
