LS19231 - Community Geriatric Nurse

We are looking for a dynamic and progressive senior nurse to review and plan care for older adults in their own homes. The post holder will be integrated within the Acute Care at Home Team preventing unnecessary admission to hospital using anticipatory care planning and current knowledge and experience of care of the older adult.  

This is a challenging post which requires highly developed communication skills, experience in dealing with a variety of different medical conditions and collaborative working within the Multi Disciplinary Team and with external agencies.  

The successful candidate will be enthusiastic and motivated and be able to work autonomously as well as in partnership with Consultant Geriatricians and GPs. They will need to possess excellent development and interpersonal skills and be committed team players.  They will be required to have highly developed or be keen to develop advanced patient assessment and clinical skills and be able to prioritise their workload effectively.  

Initially the post holder will work Mon – Friday, however this may also include out of hours working as the Acute Care at Home Team evolves.
