To be available between the hours of 8.45am and 5pm to respond to referrals, taking a key role in assessments and in the implementation of solutions to prevent unnecessary admission to hospital or to long-term care. This includes participation in the duty Social Work rota

To promote, whenever possible, service user involvement, self-advocacy, choice and empowerment.

To attend the Team's Care Management Meeting, accept appropriate referrals and carry out the tasks of Lead Professional. This will include assessment, liaison and review.

To develop effective links with families and carers in order to achieve a comprehensive assessment and to develop an appropriate care plan, based on the individual needs and wishes of the service user.

To initiate / attend multi-agency case conferences and care planning meetings when required.

To provide support to families and carers and to carry out Carer Assessments as required.

To carry out person centred assessments and to present details to the Needs Assessment Panel in an accurate and timely manner.

To encourage the development and maintenance of appropriate support networks for individuals within the community.

To be familiar with local community services available under criteria and costs.

To be familiar with Guernsey's individual benefit systems and to provide support for people on matters involving welfare rights.

To offer support and information to clients, families and carers on matters such as housing, allowances and financial matters, day care, respite care and other community services, residential and nursing home placements. To generally advise on service entitlement.

To respect an individual's independence and dignity and to offer support, as required, in the making of major life decisions and choices.

To offer support to service users and families in respect of their loss and grief.

To be familiar with, and execute, Health and Social Services Department Policies, including the Safeguarding Adults Policy.

To keep daily records of client contacts and ensure that the relevant data is entered on to the appropriate paperwork or electronic record.

To keep monthly statistics as required by the Service.

To participate in audit and the monitoring of outcomes.

To participate in professional supervision on a regular basis.

To ensure effective communication exists with the Community Team.

To maintain and develop effective working relationships in order to ensure good inter-agency working with key partners including:-

  1. Service users and their representatives.
  2. Joint service providers from Health and Social Services and other departments e.g. Housing, Social Security.
  3. Voluntary and charitable agencies.
  4. Private residential and nursing home managers.

To continue to develop and consolidate referral channels with medical staff, other main referrers and the Needs Assessment Panel.

To be aware of health and safety issues in your area of work and to report any concerns to Line Managers.

To attend staff meetings and other meetings relevant to the development of the Social Work Service.

To be involved in the Major Incident Procedure as directed by the policy.

To be aware of, and abide by, the principles of The Data Protection (Bailiwick of Guernsey) Law, concerning improper disclosure, misuse of Breach of Confidentiality in respect of information held on computer systems or otherwise.

To maintain professional standards and behaviour as specified by the General Social Care Council's "Code of Practice for Social Workers" and in accordance with local policies.

To attend relevant courses and in-service training and to share knowledge and skills with colleagues in the Health and Social Services Department.

To be willing to participate in Student Nurse / Student Social Work Training placements within the Community Teams.

To participate in the staff appraisal system.
