Learning Disability Health Liaison Nurse

An exciting development opportunity has arisen for an enthusiastic innovative and motivated Health Liaison Nurse to join a team promoting the health needs of people with a learning disability.

Working closely with colleagues you will support the education of acute health staff in the health inequalities that people with a learning disability face and will ensure that reasonable adjustments are made to support individual need.

You will be expected to work proactively with fellow professionals and other providers who provide support to the individual to assess and meet individual health needs with a focus on reducing health inequalities experienced by adults who have a learning disability. You will also be supported to develop your own clinical competency profile and to cultivate specialist areas of practice.

There are 2 part time - 18.75 hour per week posts available:

1 based in Withybush General Hospital, Pembrokeshire

1 based in Prince Philip Hospital, Llanelli

There is the possibility of combining both posts into 1 full time post with the base being at Withybush General Hospital with travel to Llanelli as required.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
