Pulse Community Healthcare has a brilliant opportunity for an experienced Learning and Disability/Autism Nurse within Birmingham. You will be working in the community and setting up specialist care packages for people with learning and disability/autism complex care needs. The role:

As our Learning and Disability/Autism Nurse, you will be predominately be home based and be able to travel across The Midlands. You will be working alongside the Head of Learning and Disability. You be responsible for setting up teams who will provide specialist packages of care for our clients, who have a diagnosis of learning disabilities and/ or autism complex needs. The care plans may formulate the transforming care agenda, where you can support clients leaving the hospital environment and getting them back into their own close communities.

You will have the autonomy to manage your own diary, in overseeing all clinical aspects of the care provisions for your clients. You will build good relationships with your clients and their families and ensure all support staff are able to perform specific clinical interventions through training, team meetings and competency sign off - all within our clinical framework:

You will ensure

  • Ensuring implementation of the 6 Cs

  • Managing own clinical caseload of clients
  • Undertaking comprehensive clinical risk assessments

  • Producing and implementing care plans
  • Reviewing and monitoring clinical aspects of care packages
  • Managing client’s clinical needs and maintaining relationships
  • Liaising with other healthcare professionals within the clients’ MDT
  • Completing support workers assessments

  • Maintaining best practise and promoting independence
  • Facilitating and educating carers within packages; including compliance
  • Developing client specific training and compliance
  • Working as part of the on call team.

About you:

You will be a Registered General Nurse (Valid NMC PIN essential) with good experience within a Learning Disability /Autism environment. Ideally, you will already have experience of working in a range of settings, for example; working in the community or forensic settings. You will need to have good knowledge of Positive Behavioural Support approaches within the community. You will be passionate about delivering the highest standard of clinical care, whilst also promoting choice and independence
