Independent Living Support Worker / Home Life Officer

Full time, 12+ hours per week

Our client have an exciting opportunity for a Home Life Officer to join them within their Care Plus Division.

You will provide individual domestic support to people in their own homes to promote independence and social inclusion.

Job requirements

  • Assessing the support needs with each customer and drawing up a mutually agreed plan. Reviewing the plan regularly in consultation with customer and Home Life Manager

  • Visiting each customer regularly to provide appropriate support as agreed in support plan

  • Liaise regularly with support officers in your locality and attend team meetings and training as required. Report on a weekly basis to the Home Life Manager to review and discuss support plan of needs for each tenant

What our client are looking for:

  • Must have a valid driving license and access to a vehicle for business use

  • Experience of working with older or vulnerable people in their own homes

  • Can work on own initiative and be flexible to respond to the needs of the service

  • Demonstrate an ability to be an effective team member
