Single Point of Referral provides one contact number for all new referrals and systematic access to community services operating a seven day service, 365 days a year during the hours of 08.00-18.00.  You will be expected to work your contracted hours on a rota basis over seven days.

We are a multi-functioning team supporting various services and work streams to include the Referral Service to community teams, Continence Home Delivery Service for patients in the their homes, Nursing or Residential Homes , and the Capacity Hub which supports the booking of beds in our four community hospitals in the county. 

Successful applicants will be given the opportunity to develop new skills and knowledge whilst working within a friendly and established team; you will be a valuable member of the team who will be supported to achieve a wide range of duties and tasks.

The successful applicant will have excellent communication skills to provide and support an efficient and effective service across the county working across the various services we provide.  You will have a pleasant telephone manner be friendly and approachable and be able to deal with conflict in a calm and efficient manner; you should enjoy working in a busy team but be able to use your imitative when necessary.

You will also need to have the flexibility to cover various admin tasks in a highly demanding work environment supporting the community nursing teams of the Trust whilst maintaining standards and following trust procedures.

You will need to be computer literate with experience of Microsoft Word and Excel and comfortable using database.

You don’t need to have worked in healthcare before, as long as you possess the above qualities and are willing and eager to learn new skills. We will provide in-house training and support.
