£59,651 to £65,185 per annum for 37 hours per week based at Lewes, East Sussex. Your starting salary will be pro rata if the above position is part-time or term-time only.

About the Role

We are looking for an experienced and professionally qualified manager to lead our specialist learning disability assessment, care management and transition teams within the Adult Social Care and Health department. This is an exciting opportunity to positively impact on the lives and experiences of adults with learning disabilities and younger people with a disability in East Sussex.

The successful candidate will have a demonstrable record of successful partnership and collaborative working, which is essential for the team’s close working with clients, carers, the local Learning Disability Partnership Board, commissioners, Children’s Services, internal and external service providers including the NHS and Independent sector partners.

The Council is part-way through two major transformation programmes with local Clinical Commissioning Groups and NHS provider trusts. For Learning Disabilities services this is embodied within the Transforming Care Programme across Sussex, which aims to re-shape and re-commission services across health and social care to deliver better outcomes for those individuals with complex and challenging needs. The postholder will be part of the shaping and implementation of these developments in partnership with senior NHS and Adult Social Care stakeholders across the local health economy, as well as being a key member of a dedicated and experienced Operations Management Team within Adult Social Care and Health.
