An exciting opportunity has arisen with the division of medicine for an Early Discharge Facilitator. The post holder will provide a comprehensive support service as part of a team both the Nursing and Medical team. We are therefore looking for someone enthusiastic and excited to take on a new challenge, develop their own skills and be involved in the continuous development of the role to provide a first class service.
You will work within the team to facilitate the smooth running of the patient journey by ensuring that coordinated and streamlined discharge planning processes revolve around the patient and their individual needs. You will work closely with all disciplines associated with a patient’s immediate medical, nursing and therapy care as well as their future care needs. You will also collaborate with other Trusts and community agencies to ensure safe, effective and timely discharges are achieved.
We are looking for individuals with excellent communication skills, the ability to work well under pressure, to ensure our patients are discharged in a safe and timely manner.
