Discharge Facilitator

Do you have an interest in, and a passion for improving safe and timely hospital discharge for patients? Do you want to gain some invaluable experience in this important aspect of inpatient care?

Yeovil District Hospital is pleased to announce the opportunity for a dynamic and proactive Band 3 Discharge Facilitator.

As the successful post holder, you will support ward staff to discharge patients (aim before midday). For example, utilising the Discharge Lounge, chasing up TTOs or any outstanding actions, liaising with families/carers, calling Care Providers to re-start care, booking transport, chasing for updates on progress, liaising with support services in the Community (i.e. voluntary sector), etc.

This post is being offered as 12 month fixed term/secondment cover.
