An exciting opportunity has arisen for a Consultant Practitioner for Older People to join our Frimley Health Team. We are looking for an exceptional senior nurse or allied health professional that is experienced, innovative and committed to excellence.

The successful candidate must have: substantial clinical knowledge in medicine for older people; held / hold a senior leadership position; and extensive nursing or allied health professional experience, specifically working with older adults, living with Frailty and / or multiple comorbidities. The candidate must possess a proven record of: clinical expertise; leadership and consultancy; delivery of education and training; continuing professional development; and service development and improvement.

It is essential that the post holder is resilient, politically astute and an excellent communicator both spoken and in written word. The right candidate will have strong interpersonal skills with the ability to command credibility.

The Consultant Practitioner for Older People will be responsible for providing and ensuring exceptional clinical care to patients, leading and developing the Advanced Clinical Practitioner role within Older People’s Medicine and working collaboratively with heads of services, medical colleagues and the Frimley Integrated Care System to continually improve quality, safety, outcomes and efficiencies for older adults. This will include the provision of:

Expert practice: To deliver high quality care to patients, enabling other practitioners to maintain professional expertise, and exercising a high degree of personal autonomy; spending at least 50% of time in direct contact with clients, patients or communities.

Education, training and development: To develop a learning culture across the system to: develop staff potential, grow and transform the workplace, enable learning, developing and improving in and from practice to promote excellence.

Professional leadership and consultancy: To lead and influence the strategic development of person-centred, compassionate, safe and effective care and the on-going transformation in changing contexts; while facilitating collective leadership capability and capacity.

Practice, service development, research and evaluation: To Lead and facilitate the development of professional practice, through the promotion of evidence-based practice, audit of standards of care with a role in research and evaluation of practice within specialist fields.
