As part of the Stoke Community Directorates transformation plan a number of posts have been developed, to support a locality-based model of care within Stoke on Trent.
We are looking for an enthusiastic and highly motivated Community Mental Health Practitioner to join our Community Mental Health Team at the Greenfields Centre in Tunstall, Stoke-on-Trent.
This newly developed post will be based, within a multi-disciplinary Community Mental Health Team, working closely with a number of professional groups.
Your primary role will be to undertake initial holistic assessments, which will include evidence based risk assessments and the formulation of care with service users. You will have expertise in crisis and risk management, together with good communication skills.
As part of an Outstanding Trust (CQC) you would be working in a highly-motivated community multi-disciplinary team. There will be opportunities and support for continuing professional development and to actively participate in quality innovation, research and audit. There will also be opportunities to lead on particular projects for the team and influence team development and care pathways.
