To have continuing responsibility for a defined caseload and provide direct clinical care to patients and support to families with highly complex needs where there is a diagnosis of cancer or other life limiting condition.
To provide clinical expertise in symptom management and specialist palliative care, support and counselling to patients who are suffering from cancer and their families.
To act as a specialist palliative care educational resource to all members of the multi-disciplinary team in a defined locality, contributing and collaborating in the delivery of evidence based care
To enhance the provision and development of palliative/cancer services within NHS Lanarkshire and across all health care settings in the attainment of the highest standards of clinical care
To provide bereavement support for carers and specialist advice to other professionals and agencies on bereavement related issues.
