Community Learning Disability Nurse

The Community Team for People with Learning Disabilities delivers specialist multi-disciplinary healthcare assessment and treatment services locally for children and adults who have a learning disability presenting with physical health needs, a mental illness and / or severe challenging behaviour. The service provided is person centred and tailored to the individual and their environment, taking into account their ethnic and cultural backgrounds and needs. We provide this through direct support, joint working with other services and / or through offering consultancy to other professionals involved.

The team works in close partnership with the service user, family / carers, and other agencies e.g. generic Health Services, independent and voluntary services, Advocacy Services and Commissioners.

The role of a Community Nurse will require you to work collaboratively to provide specialist assessment and intervention to people with learning disabilities in the community, working with a range of issues and complex needs.

The Community Team for People with Learning Disabilities works flexibly but mainly from Monday to Friday 09.00 – 17.00 proactively supporting individuals to maintain their physical and emotional wellbeing in the community.

We are looking for a motivated and enthusiastic individual with a passion for improving the lives of people with a learning disability. You will need to demonstrate genuine commitment to working with our service users to improve their health and wellbeing outcomes.

You will be based at Carey Suite, St Mary’s Hospital, Kettering but the ability to travel across Northamptonshire is essential.
