Community Learning Disabilities Nurse

We are looking for Social Workers and Community LD Nurses that have experience with LD and/or Mental Health. preferably Adults but Children’s isn’t ruled out. Knowledge of complex needs, CHC and safeguarding is a requirement and good report writing skills are essential. It’s a community team with flexible working (within reason!).

The Adult Learning Disability Team is an integrated service consisting of qualified social workers and community learning disability nurses.

Integration is about commissioning and providing responsive services for people who need them in the least restrictive and appropriate manner.

Services are provided to adults from the age of 18, who have a recognised learning disability and the individual and/or their carer meets the agreed eligibility criteria.

The adult learning disability team focuses throughout on what people can do, with support where necessary, rather than on what they cannot do.

The service operated by the integrated team is based on the principles of valuing people now;

The integrated model of services offers;

• A seamless and person centred service
• Safeguarding of vulnerable adults
• Supports local and national strategy
• Improves day to day and organisational communication
• Links organisational structures
• Creates a multi-disciplinary approach to decision making and problem solving
• Strategic planning will be influenced by an integrated service.

Purpose of role

  • To carry continual responsibility for the management of a caseload.
  • To promote the needs and rights of people with learning disabilities with other agencies and the general public.

  • To contribute to the development of new services and projects within the community team.

  • To participate as an active member of the integrated adult learning disability team and establish and maintain effective communication with other agencies and professionals, to determine good outcomes for people with a learning disability.

  • To work with service users to assess their needs, identify options for meeting those needs, negotiate and review appropriate care within the Department’s priorities. To work with service users and/or liaise with service providers from within the Department or other agencies, as necessary.

  • To participate in secondment opportunities to our Care standard review team, as part of the ongoing care management process of review and monitoring support packages.
