We have an exciting opportunity for a specialist nurse with ITU/HDU/Complex Care experience to join our team of community nurses within Cardiff.

As a member of the community nursing team you will assist our clients to live an independent life within their own homes. As a specialist community nurse you will be travelling around our community packages completing clinical reviews with your allocated clients, undertaking practical competency assessment with our support workers of which the training will be in line with Pulse Community Healthcare’s policies and procedures.

You will be involved in the following

  • Completing clinical assessments and client specific care plans for new package referrals

  • Delivering person centred care and supporting the needs of the client within their own home and the community

  • Regular client clinical reviews

  • Working alongside the local branch to ensure the competency of support workers are assessed and met

  • Using your vast knowledge and skills, you will prepare community support workers to transition into competent members of the team

Essential Skills

1 year recent experience in ITU/HDU/Complex care setting

  • Leadership ability

  • Adaptable to a variety working conditions

  • Registered Nurse with a current NMC registration

  • Willing to travel frequently

  • Flexible Working Approach

  • Current DBS Check

  • Experience of managing client relationships

  • Proof of indemnity insurance (this may be demonstrated via membership of the RCN or other relevant professional body)

  • Desire to work in the local community

  • Excellent verbal and written communication skills

  • Ability to use own initiative and be comfortable lone working as well as part of wider team

  • Be able to adapt to working within the commissioned service level agreement

  • A good understanding of professional boundaries

  • Knowledge of safeguarding and person centred care

    Desirable skills but not essential

  • Experience in community complex care

  • Mentorship/teaching certificate
