Duties and Responsibilities:
*To formulate and design appropriate family plans to meet identified needs and to arrange for the implementation of such plans, in conjunction with other appropriate agencies.
*To provide appropriate ongoing support to individuals and families as the key-worker.
*To ensure that individual professional standards are maintained in line with agreed levels, including full compliance with policy and procedural guidelines and accepting primary responsibility for the level and quality casework recording.
*To feed back to the Team Manager any problems in relation to the effective provision of services and/or policies.
To apply for this role you will have:
*A recognised Social Work qualification, e.g. DIP SW/BA in Social Work.
*Must have a full driving license and use of a car.
Along with Knowledge and an Understanding of:
*Departmental policies and procedures, including the Equal Opportunities policy relevant to the role.
*Ability to acquire and implement relevant knowledge.
*Understand lines of accountability.
*Evidence of practice placement or work in a related setting and ability to articulate this experience with service users.
*Effective inter-personal skills.
*Sound organisational skills.
*Ability to undertake assessments of service users and carers.
*Develop plans and implement care packages
*Work purposefully in partnership with service users and their families.
*Ability and knowledge of IT systems and the LCS systems.
*Able to respond to regular supervision and support.
