Pulse provides safe, effective social care and health support services to people in their own environment. We work as a trusted partner with a variety of organisations to provide 'care packages’ in the community. These include hospitals and primary care trusts, local authorities, GPs, charities, case-management companies and voluntary organisations as well as private individuals

Pulse support people with a variety of specialist care and nursing needs. Our aim is to make a positive difference to the well-being of individuals and where possible improve their wellbeing. We work with over 200 commissioners nationwide and work in partnership with clients and commissioners to design very specific and personal clinical care plans to enable our clients to live as independently as possible, encompassing their likes, dislikes and preferences wherever possible.

To do this, we require an organised and passionate Case Manager to join our friendly, hard working team within Bristol

This role is permanent and full time and you will be based at our South West branch. The role will require you to manage your own case load of clients and will be a mixture of working within the community and within our Pulse Community Healthcare office in Bristol city centre.

Reporting to your Registered Manager, the Case Manager is responsible for providing the agreed service to clients by directly managing their care packages through the skilled support workers within that package. This will be both from the office and within the community as the package is designed to assist individuals to live within their own homes and communities whatever their abilities through a delivery of person centered care and positive behavioral support.

The role is a very 'hands on’ operational role which will require someone with a very positive and resilient attitude towards work. The role of Case Manager will include-

  • The development of person centred care plans- promoting client independence wherever possible.

  • Effective management of care packages to ensure high standards of care are achieved in each service delivery

  • Taking an active role in the recruitment and retention of support workers and to ensure all staff have knowledge of current policies, procedures and guidelines in use within the Company

  • Assistance with audits, compliance regulations and all relevant quality policies and procedures, both internal and external.

  • Keeping abreast of current commissioning developments regarding the community healthcare sector.

  • Escalating any issues to the Regional Manager quickly and effectively

  • Communicating effectively with Commissioning Managers and multi-disciplinary teams as well as service users and their families

Although the job provides consistency within your case load, the role is also diverse and varied, allowing you to grow and develop in a safe environment. As an ideal candidate for this role, the following is essential -

  • Experience in the development of person centred care plans - hands on care experience would be desirable

  • Strong interpersonal skills with the proven ability to prioritise and organise case loads effectively, within a pressurised environment

  • Excellent communicator at all levels, able to demonstrate strong people / client management skills

  • Be resilient and able to adapt to any changes whilst continuing the same excellent level of care across the board

  • Car owner and driver due to level of community work

  • Confident and passionate individual to encompass our strong company values
