Care Sourcing & Placement Officer

This role for a Care Sourcing & Placement Officer has been released on a temporary basis for 1 year initially.

Main Duties:

* Assist and support the Senior Care Sourcing & Placement Officer to deliver a high quality contract management, care placement and governance function within the scope of the Council's procedures and Financial Regulations
* Act as a contact point for related services inclusive but not limited to Continuing Health Care, the Reablement Service, Hospitals, Supported Hospital Discharge service, Crisis Response and Operational Teams in order to facilitate effective and efficient whole system flow through the client pathway process.
* To make arrangement for placements and support on behalf of others by
a. Receiving referrals for service
b. Signposting and providing simple information and advice to enable service users to make informed decisions on their support needs
c. Identifying cost-effective support options within identified budgets
d. Liaising with service users and their families/informal carers to offer cost-effective choices and options for support
e. Sourcing appropriate care services as required, keeping customers updated on progress at appropriate times
f. Negotiating on price where appropriate to ensure the council receive best value for money
g. Ensures the completion of associated contract governance documentation to a high standard in line with specified delegated responsibility
h. Confirming the commencement of care/support services within placement target times
i. Liaising with internal commissioners and operational staff to update on progress
j. Liaises with other Quality & Contracts staff (and others) to problem solve difficult placement cases
k. Feeds back to referrers on the final outcome of placement actions
* Ensure that users, carers and families are at the centre of the work undertaken with good communication at all times.
* Ensures the robust administration and accurate recording of placement information and documentation on the council's ICT systems
* To support, develop and maintain good relationships with suppliers, and work with colleagues across the Joint Commissioning Team to establish and maintain an effective and consistent dialogue with the health and social care market, and as appropriate regionally and nationally.
* To contribute to an overall team approach and recommend ways in which the service can improve.
* Ensure that children and vulnerable adults are safeguarded at all times and in line with current legislation, and where appropriate to report areas of concern.

You should match the following criteria:

* Qualified social care, education or health professional, or equivalent (e.g. voluntary sector/housing)
* Purchasing & Contracts related and/or Care Commissioning and/or Financial and/or HR qualification by examination or working towards this.
* Basic understanding of commissioning and contracting in the social and health economies
* Basic understanding of the legislative frameworks for safeguarding vulnerable adults and children
* Knowledge of local government and public sector practices in relation to Care quality, and contract monitoring.
* Ability to assess and interpret customer and supplier information to determine performance achievement.
* Understands how to design and implement a system for contract monitoring
* Understanding of Care Quality Standards
* Broad understanding of health and social care issues and The Care Act
* Understanding of Contract Procedure Rules
* Outline knowledge of tools used to commission services effectively
* Experience of contract management and/or financial management and/or purchasing or HR procedures in the public or private sector
* Experience of working jointly with partners both providers and commissioners.
* Experience of working in a complex and adaptive environment
* Experience of supporting vulnerable people, either in a professional or personal capacity.Desirable:
* More than one year of experience of commissioning services in the public sector
* Experience of managing budgets
* Experience of contract management
* Application of CPR's and other relevant legislative frameworks
* Experience of working directly with users and carers relevant to the portfolio
* Risk management and safeguarding legislation and guidance relating to social care governance and contract compliance
* Experience of participation in a tendering process through to award of contract
* Experience of improving poor performance or systems
