Position: Head of Care - Nailsworth

Location: Nailsworth, Gloucestershire

Salary: £30,000 - £35,000 per annum

The core working hours for this role will be office based, however the post holder will be required to work in the community; at another office, as requested by the Registered Business Manager to meet the business needs. This role is not home based.

Responsible for overseeing the day-to-day running of the field-based Care Management team.

Auditing & Quality Assurance in line with CQC regulations.

Report to the Business Manager on a regular basis.

Supervise and conduct annual appraisals and supervisions for the field-based Care Management team.

Care Management of a national client base.

Work closely with the Client Services Manager and Bookings team to ensure quality client contact is maintained at all times.

Manage and schedule the on-call rota.

Out-of-hours schedule and phone duty when required.

Key Duties:

To ensure effective and responsive line management of the field-based care management team.

Ensuring that standards are meeting CQC requirements.

Take accountability for client and carer compliance ensuring CQC regulations, company policies and practice are maintained and promote our aims and values.

Responsible for quality assurance and monitoring of field-based care managers.

Promote, drive and grow brand new care packages.

To be responsible for leading the investigation and resolution of any concerns or complaints from clients or carers in accordance with Complaint, Safeguarding and Whistleblowing policies. Providing direction to Care Managers as required.

To implement new care policies or procedures as directed by Senior Management.

To provide weekly support call meetings with field-based care managers and support as required.

To lead monthly Care Managers meetings

To liaise with medical and non-medical staff external to as required, e.g. Social Services and hospital or nursing home staff.

To oversee and manage a national client base to include conducting care and risk assessments and subsequent reviews.

Following assessments and reviews, ensuring paperwork and information update on staff plan is completed in a timely manner.

To act as an ongoing point of contact for clients to ensure the service provided meets their needs and respond to any subsequent matters.

In co-ordination with the Client Services Manager, provide regular telephone support to clients and their relatives.

In co-ordination with the Bookings team, provide additional information to care workers prior to client placements.

To provide support and care guidance to carers if needed during an assignment.

To prepare KPI’s to deadline.

To deputising for the Business Manager - in their absence.

To be responsible for maintaining and updating the Care Management Procedures - to be reviewed as a minimum once annually.

To plan induction programmes relevant for new care managers

To be responsible for scheduling the on-call rota and advising the team of required duties.

To be responsible for ensuring all staff members are fully trained before they participate in the on-call rota.

To maintain and develop the on-call procedures - to be reviewed as a minimum once annually

To cover the out-of-hours evening and weekend phone duties as and when required.
