Position: Care Coordinator
Location: Swansea
Hours: Part-Time and Full-Time Positions Available
Main Purpose:
To enable Service Users to remain in their own homes by providing the highest standard of support and care required to help them achieve maximum independence whilst maintaining their self-respect and dignity
Main Duties
- Adhere and implement all Company policies and procedures, including Equality and Diversity, to ensure the aims and objectives of the organisation are met
- To assist the branch manager in all aspects of care management in the designated area
- Produce data for the accounts department as required
- Assist with problems that may arise between Care Workers and Service Users effectively on a daily basis
- To act at all times in a professional manner as a representative of the Company and to support its good reputation
- To manage a team of Care Staff trained to carry out personal care and practical support
- To co-ordinate Care Staff and resources & monitor individual care packages whilst matching Care Staff to Service Users to provide a person-centred service
- Ensure the efficient and effective delivery of services to all Service Users
- Liaising with Social Services and all effective social care agencies, working efficiently to provide effective support to clients
- To compile and maintain accurate records and ensure they remain up-to-date on at least a daily basis
- To ensure the service delivered meets the requirements of the Service User
- To ensure that all requirements of Health and Safety legislaton are met and the Registered Manager is informed of any incidents that have arisen
- To raise awareness of the complaints procedure and where action is necessary, report immediately to the Registered Manager.
- To partcipate in the out of hours, weekend and Bank holiday 'on-call rota’
- Ensure all associated administraton is in place through liaison with the office staff
- Maintain regular contact with Service Users by telephone to regularly monitor and review the effectiveness of the care package
- To respect Service User and staff confidentiality in accordance with Company policy & the Data Protection Act
- To be accountable for your work practice and take responsibility for maintaining and improving your knowledge and skills by attending ongoing training as requested by the company
- To assist the Care Manager in identifying any training requirements needed to promote staff self-esteem and development
- To follow the complaints procedure in cases of dispute and to report all accidents/incidents, no mater how minor, to the Registered Manager
- Ensuring that holiday/sickness and emergency calls are assigned
- To assist Care Manager in the development of the staff team to ensure that the needs of the Service Users are met
- All other appropriate duties that fall within the purview of the post as may be agreed
