The care coordinator is responsible for the continuity of care for all our clients. This vital role has a huge impact on the lives of many people who rely on our care and support. Clients tell us that what really matters to them is having a stable team of caring and professional care workers whom they can rely upon along with a supportive and organised office team. Working alongside the registered manager/owner, the care coordinator will be responsible for making this happen. The ability to problem solve and think clearly when under pressure is essential for this important role in our company. You will use your excellent planning, organisational and IT skills to make sure that each client receives the care and support that they expect on time. This involves:

  • Allocation of care workers to client visits by using effective rostering (People Planner and Mobizio), ensuring continuity of care, considering locations, skills required and client preferences.
  • Organising care worker rotas to minimise changes to the service and travel between client’s homes
  • Responding efficiently to day to day changes in the care and support packages
  • Taking responsibility for new referrals from private clients and social services teams, ensuring full information is received and appropriate care planning is undertaken, growing the business in line with the business targets
  • Liaising with Social Workers, OT’s, GP’s, District Nurses and other professionals to ensure the optimum health and well-being of the clients.

The role offers the opportunity for developing positive working relationships with clients, their families, professionals and our care team. You will provide a crucial link between each client and their care workers. You will often deal with queries from clients about their care and support. This requires a sensitive understanding of client needs and a supportive and confident telephone manner.

The care coordinator must also be clear about the best way to work with care worker schedules to maintain continuity in the delivery of care. You will need to build strong relationships with the care team, you will need to call upon them for extra cover when needed. Care workers work over the weekend and evenings, so the post holder will be part of the on-call emergency team rota.


You must demonstrate commitment to working within a team, have a flexible approach to working hours and experience within the care industry, preferably home care. This role will involve an element of on-call duties, therefore a driver with access to your own vehicle is essential as you will be required to cover care calls at times.You must be a quick learner and be able to work in a fast-paced environment.
