Care Coordinator – Domiciliary Care
40 hours per week on-call | £28,000 (rising post-probation)
Our client, a well-respected and growing provider of domiciliary care, is excited to be opening a new domiciliary care branch—and they're looking for an experienced and compassionate Care Coordinator to play a key role in its success.
This is an exciting opportunity for someone who thrives in a fast-paced, people-focused environment and wants to help shape a service from the ground up. The successful candidate will be central to delivering safe, high-quality care that enables people to live well in their own homes.
As a Care Coordinator you will work closely with the Registered Manager to ensure care packages are planned and delivered effectively, schedules are well-organised, and clients receive the consistent, person-centred support they deserve
.
Due to the nature of the role, candidates must hold a full driving licence, as regular travel within the community will be required.
Required Skills & Qualifications
Level 3 (or above) in Health and Social Care
Previous experience in the care sector – ideally in a Care Coordinator or team leader role
A confident communicator with strong organisational skills
A proactive problem-solver who can manage changing priorities
Full UK driving licence and access to a vehicle

Key Responsibilities
Meet regularly with service users to review and ensure care aligns with their individual care plans
Train and assess care staff in the safe and effective administration and management of medication
Coordinating care visits and rotas to meet service users’ needs
Managing client referrals and care planning alongside the Registered Manager
Supporting and supervising care staff, including conducting spot checks and field visits
Handling on-call duties on a rota basis (shared within the team)
Ensuring the quality and compliance of care provision in line with CQC requirements
Building strong relationships with service users, families, and professionals
