Band 4 Medical Administrator/Co-ordinator

I am currently recruiting for experienced assist the Care Co-ordinator in acting as the single point of contact for referrals into Service. The post will include working with Service Users to identify goals and planning care.

Location: Cambourne

Temporary: Expected to last 3 months with possible extension

Hours per week: 37.5 hours per week

Salary: £11.00 to £11.50 (Umbrella) or £9.85 to £10.35 per hour (PAYE)

  • Highly organised with administrative experience in a healthcare setting

  • Accurate and quick data input

  • IT literate with competency in Microsoft applications

  • Excellent telephone manner

  • Flexible approach to working

Duties will include but not limited to the following:

  • First point of contact for referrals into the Re-ablement Service Liaise with professionals and the assistant team manager within the team to refer for allocation, where appropriate, for comprehensive needs assessments of service users

  • Assist with instigating charging process as appropriate and refer to care manager

  • Assist with liaison with Care Purchasing Co-ordinator to instigate long-term care requirements

  • Record contacts on Health and Social Services databases to maximise information sharing and ensure smooth pathways through services

  • Assist with monitoring the progress of referrals, ensuring the two week review and any subsequent reviews are initiated and completed

  • Draw up staff rotas electronically to ensure adequate skill mix and cover for all shifts

  • Where necessary, explain the service provision to the user and make an essential initial health and safety assessment

  • Assist with ensuring that care plans are implemented

  • Undertake staff development activities such as NVQ assessments and induction of new care staff
