We are looking for an enthusiastic and motivated individual who is committed to learning and developing within our team. You will work closely with the other Specialists Nurses to deliver high quality care to patients with specialist palliative care needs. You will need to demonstrate excellent communication skills in supporting patients and their families, and liaising with members of the multi-disciplinary team.
Working as part of an integrated team of nurses, within the community setting to provide a responsive service, providing specialist palliative care advice including the ability to deliver safe evidenced based symptom control advice, alongside the provision of social, emotional and spiritual care for the patient and family. You will work closely with, and act as, a resource to primary care teams and other care providers with a common aim of achieving the best quality of life possible for our patients.
