The Role: To effectively develop and manage a team of staff in providing person-centred support to individuals with a learning disability, thus enabling them to live within their own home, increasing their independence and decision making and ability to actively participate in their community.

Shortlist Criteria:

- Have a full UK driving licence and/orown a car would be an advantage

- Flexible to work 24/7 rotating shift pattern including nights

- Experience of working with people with a learning disability

- Experience of supporting people to plan their future and their day to day support

- Experience of working with people who have complex health or behavioural support needs

- QCF level 3 in Health and Social Care or equivalent or a commitment to achieving this within 12 months

- Knowledge of the Mental Capacity Act and its practical application

Job Description

RESPONSIBILITIES:

- Promoting and facilitating a person-centred culture

- Actively, contribute to the standard of the support provided

- Act as a positive role model for other team members

- Assist the Community Support Manager in monitoring compliance

- Observing and monitoring practice to assure the quality of support provided

- Coaching and developing team members through practical day to day support 'hands-on’ including through induction and probation

- Take a lead role in ensuring individuals are supported to make decisions following the principles of the Mental Capacity Act

- Assist the Community Support Manager in the management of the team including the development of the rota and deployment of staff

- Assist the Community Support Manager in the training, supervision and appraisal of Support Workers including Relief Support Workers

- Assist the Community Support Manager in monitoring individual’s finances

- Attend relevant meetings on behalf of the Community Support Manager

- Develop and sustain relationships with external agencies

- Undertake all duties of a Support Worker

- Ensure compliance with Perthyn’s policies and procedures

- Undertake any training and development initiatives identified by the Community Support Manager

- Contribute to effectiveness of the team

- Undertake any other duties commensurate to the job role
