Ideal candidate would have a qualification of Foundation Degree in Health and Social Care or working towards one, with established relevant experience. They would have excellent clinical knowledge and proven communication skills. Working as part of an established team, the candidate would be able to demonstrate a positive and enthusiastic attitude towards quality improvement and evidence experience of working to high levels of confidentiality. Experience within the Primary Care setting would be an advantage, but not essential, providing the candidate can demonstrate transferrable skills. They would work collaboratively with the GP, Nursing and Management teams to meet the needs of the patient and sustain the strong ethos of the Practice.
