Our Health Reablement Teams are seeking enthusiastic and dedicated Assistant Practitioner with a sound knowledge and experience of working with complex, frail and older people.

Citycare’s Health Reablement services deal with a wide range of cases, providing care and rehabilitation for people presenting with a plethora of multi pathologies, as well as challenging medical and social circumstances.

We are seeking an Assistant Practitioner with a wide range of holistic health and social care skills and experiences to complement our service. The team is dedicated to providing rehab and assessment for patients who have been stepped down from hospital or stepped up from the community into a CityCare Community bed. You will be working closely with Occupational therapists, Physiotherapists and Nurses as well as GPs, District Nurses, Community Matrons, the acute trust and a range of Health and Social Care Professionals.

The aim of our service is to facilitate timely and safe discharges from the community beds into the appropriate discharge destination. We promote independence and through rehabilitation and assessment, we strive to help our service users to achieve their goals, however big or small.

Providing holistic assessments and a range of skills within the AP competency framework, the Assistant Practitioner will be expected to complete initial assessments and follow ups within their competency framework and under the guidance and supervision of the Clinical staff.

The hours of work will be 8am- 10pm on a Rota basis. The Service operates 7 days per week, 365 days per year
