The purpose of this role is to ensure the delivery of a high quality patient focus, working in partnership with the MDT to facilitate a high quality of care and discharge planning. Positions are available to start ASAP on an ad-hoc, full time or part time basis. We are also offering all new registrations a bonus of £150.00
