Advanced Nurse Practitioner – The Humbleyard Practice

The Humbleyard Practice is situated south of Norwich. It compromises three surgeries based at Cringleford, Hethersett and Mulbarton.  We provide general medical services to just under 20,000 patients and are partially dispensing.

The clinical team consists of 7 GP Partners, salaried GPs, ANPs, ECP, Outreach Nurses, Practice Nurses and HCAs.  We also have a full managerial and administrative team across all three sites.  We are active members of Ketts Oak Primary Care Network, a group of general practices who work together to delivery high quality primary health care to their population.

The successful applicant will be qualified and experienced in working as an independent practitioner and prescriber with minimal supervision. Appropriate qualifications and experience to enable the successful candidate to examine, diagnose, treat, prescribe and refer patients presenting with acute illness and undiagnosed and undifferentiated conditions are essential.

The post holder wil be an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient including clinical assessment, diagnosis and treatment. They will demonstrate safe clinical decision-making and expert care for patients in the general practice. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Job responsibilities

The Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice providing acute care for patients via telephone triage, video consultations and face-to-face appointments. The ANP will need to provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care, making any necessary referrals for investigations in the appropriate manner.

Essential Qualifications:

RGN qualification, BSc Nursing Practice

Nurse Practitioner Pathway

NMC registration

Prescribing qualification

Fulfill the NMC post-registration education and practice requirements

Evidence of a portfolio that shows that individual is  educationally active and have done the required updates in all areas of competency

It would be desirable if the applicant had experience in the following:

Telephone triage and video consultations

Management of long term conditions such as COPD and asthma

Palliative Care

The post is flexible - full or part time
