Are you an experienced advanced clinical practitioner?
We are looking to recruit an enthusiastic, forward thinking Advanced Clinical Practitioner to work within the Community React Team (CRT). The post holder will work as part of the Community multi-disciplinary teams to manage patients in the community setting, including the patient’s home, clinics and the Community beds assessment area at the acute hospital site. The post holder support the clinical team in terms of clinical leadership and expertise.
The CRT works in collaboration with North West Ambulance Service (NWAS), Primary Care, Care Homes and the wider healthcare economy, with a focus on safe admission avoidance and supporting patients in crisis by managing their care and treatment in their place of residence. The team also have a key role with patients being discharged from hospital to ensure safe effective discharge.
This is an exciting time for the Community React Team as we continue to evolve, working closely with our key stakeholders. The expectation is to work within a changing environment and to drive the transformation agenda.
The service is operational 7 days per week from 8am-10pm .
Applicants may hold professional registration from a Nursing, or Allied Health professional background.
Please Note: Candidates will be asked to present original qualification certificates to the interview panel.
Based in the Community division so you will need to be required to travel across the borough.
