Are you an experienced, enthusiastic Adult Social Worker looking for a fresh challenge?

If so, then we have the perfect opportunity for you.

Here at Medicarefirst, we are recruiting an Adults Social Worker to join the All Ages Disabilities Team in the Redcar and Cleveland region.

The pay rate for this position is up to £25 per hour.

Responsibilities include:

*Undertake assessment of need for adults with Learning Disabilities and/or Autism.
*Undertake assessment of need for Children with Learning Disabilities and/or Autism via the Transition of care process.
*Undertake review of need alongside review of existing support plan for Adults and children with Learning Disability and/or Autism.
*Arrange services to meet identified need.
*Provide joint working with social care colleagues working in the Forensic Learning Disability team.
*Hold a recognised qualification in Social Care and appropriate professional registration.

Experience preferred:

*A commitment to multi-disciplinary and partnership working with social workers, health colleagues, providers; and clients their carers and families.
*Experience of working in a similar role or with people who experience Learning Disability and/or Autism.
*An understanding of how to work efficiently and effectively in challenging circumstances.

Minimum Requirements:

HCPC Registration
1 year post qualifying experience within the UK Social Care
Enhanced DBS/CRB (we can obtain for you)
Eligibility to work in the UK
