You must have a degree in Social Care or equivalent and be HCPC Registered.
You will be responsible for managing the Adult Social Care Duty Team. The Duty Team works in collaboration with Brent Customer Services to manage the Adult Social Care “Front Door” service. There are five other permanent members of the Team who you will line manage – two Social Workers and three Duty Officers - and an interim Occupational Therapist.
The Team is responsible for undertaking initial social care assessments and determining eligibility of clients in-line with Care Act eligibility criteria; putting in place interim packages of care (required within 24 hours); conducting welfare visits where required; dealing with referrals from private and non-acute hospitals and A&E and rehab and reablement services; implementing increases and decreases in care packages within set parameters where changes are required. The work of the Duty Team is complex and varied.
