This is an excellent opportunity to work within a committed, multi-disciplinary environment that supports adults experiencing mental distress across the Torfaen area.Adult Services works collaboratively with partner agencies to promote choice, control, positive risk taking and meaningful social inclusion, supporting individuals to achieve positive recovery outcomes in the areas that matter most to them.Key ResponsibilitiesDeliver high-quality social work practice to adults experiencing mental health challenges.Undertake comprehensive assessments, care planning and risk management.Work proactively with individuals to promote independence, wellbeing, and recovery.Collaborate closely with multidisciplinary teams, partner agencies and hospital-based professionals.Support individuals in both community and hospital settings.Maintain accurate records and prepare reports in line with statutory and organisational requirements.Participate in team meetings, case discussions and service development activities.What We’re Looking ForRegistered Social Worker with Social Care Wales (or equivalent registration).AMHP qualification is desirable – if not already held, the successful candidate will be expected to undertake AMHP training (fully supported by TCBC).Strong understanding of social care processes within adult mental health.Excellent assessment, risk management, and decision-making skills.Flexible, enthusiastic, and dedicated to supporting individuals experiencing mental distress.Ability to work effectively within a fast-paced, multidisciplinary team environment.Commitment to promoting recovery, autonomy, and positive outcomes.Additional InformationThe AMHP qualification attracts an additional market supplement when part of the AMHP rota (details available on request).You will join a supportive team that offers excellent development opportunities and encourages ongoing professional growth.Work LocationCounty Hospital, Pontypool Pontypool NP4 5YA United Kingdom
