Complex Care Professionals specialises in delivering complex homecare in service users homes. We pride ourselves on putting our service users first and protecting their safety and dignity. We create bespoke care plans that address both the medical and non-medical aspects of each clients wellbeing providing them with friendly and compassionate support.The role of the Nurse will be based in our Rochdale branch and will require visits to our service users' homes. The responsibilities of the Nurse will include but not limited to the following: Provide clinical leadership and support to care staff across the branchAct as the clinical lead for complex care packages delivered in service users’ homes.Promote safe, effective, and person-centred care at all times.Undertake initial and ongoing clinical assessments of service users in their own homes.Develop, review, and update individualised care plans and risk assessments.Ensure care plans reflect changing clinical needs and promote independence and dignity.Liaise with service users, families, and multidisciplinary teams to coordinate care.Administer and supervise complex clinical procedures (e.g. PEG feeding, tracheostomy care, spinal injury care including bowel management and bladder washout, NIV, Cough assist).Provide guidance on infection prevention and control.Escalate clinical concerns promptly and appropriately.Conduct clinical audits and contribute to service improvement plans.Ensure accurate and timely clinical documentation.Maintain effective communication with service users and their families.Act as a safeguarding lead or support safeguarding processes within the branch.You will need a full UK driving licence and access to your own car.
