Working closely with Senior Sisters, GPs, therapists and wider multidisciplinary colleagues, you will help ensure patients receive safe, effective and compassionate care tailored to their individual needs. You will manage and prioritise your own workload, responding to changing clinical needs and ensuring patients and carers remain fully involved in decisions about their care. You will also play an active role in health promotion, supporting people to improve their wellbeing and maintain their independence wherever possible. Alongside your clinical responsibilities, you will contribute to service improvement initiatives, clinical audits and the ongoing development of community nursing services, helping us continue to deliver high quality care across Gravesend.
